We are currently hiring a Hepatitis C Patient Navigator, to work in our Care Coordination Services Office. As the Hepatitis C Patient Navigator, you would be responsible for:

• Screening and Testing for Hepatitis C

• Linkage and Navigation to Hepatitis C treatment

• Hepatitis C education

• Harm Reduction Support

Hepatitis C Patient Navigator Minimum Salary: $20.00

Hepatitis C Patient Navigator Maximum Salary: $20.00

Hepatitis C Patient Navigator Schedule: Mon-Fri 8:30-4:30

Qualifications:

• Associate Degree

• 1-3 years’ experience with target population
