Johns Hopkins University Patient Navigator in Baltimore, Maryland

The Adolescent Medicine division of Pediatrics is seeking a full time Patient Navigator. The division is looking for someone with previous peer mentoring experience with persons experiencing substance use and substance use disorder intervention/treatment or behavioral health challenges. They successful candidate will facilitate access to care through navigation services, peer support, community outreach, and will help clients assess barriers to follow up or adherence to the care plan. The peer navigator will be available to escort clients to appointments, including helping with off-site services. This is full-time position, offers a five-day work week, generally Monday-Friday work week. The schedule may include some periodic weekend support/planning of program activities but will otherwise consist of a weekday schedule determined in conjunction with the Supervisor.

Specific Duties & Responsibilities

  • Engage and develop rapport with our primary care and specialty care population and other key community members with the goal of entering into peer navigation relationships that can help invite adolescent and young adult patients into services, utilizing relevant personal experience, trauma informed practices and person-centered methods.

  • Assist adolescent and young adult patients as well as their parents, family members and appropriate community members with short-term access to program and community services and access to referrals and other resources available in the target area.

  • Timely data entry necessary for tracking program goals, outcomes, assessments, referrals, and other information as necessary.

  • Helping adolescent and young adult patients and family members acquire transportation, and, as necessary, to appointments for medical, behavioral health, benefits, or other necessary services.

  • Liaise and coordinate supportive services within the JHMI and from other organizations who are part of the care network for services for vulnerable youth including substance use prevention, early intervention, treatment, and support services.

  • Leverage peer navigation experience with other community peer navigator programs to help build and expand one’s skill set and to improve this model of service.

  • Check-in with clients on a regular basis to ensure support and engagement around care.

  • Assist with transportation and help develop support and engagement for youth as well as help with data management and maintenance of the data collected as appropriate.

Minimum Qualifications

  • High School Diploma or graduation equivalent.

  • Two years related experience.

  • Additional education may substitute for required experience, to the extent permitted by the JHU equivalency formula.

Preferred Qualifications

  • Previous peer mentoring experience with persons experiencing substance use and substance use disorder intervention/treatment or behavioral health challenges and/or Certified Peer Recovery Specialist Certification preferred.

  • Ability to manage conflict, mitigate/respond to stressful situations and engage traumatic stories.

  • Ability to understand and maintain professional boundaries.

  • Ability to listen with care, treat patients/families with respect and dignity, and respond to each individual’s unique needs.

  • Willingness to participate in ongoing institutional and other trainings to complement the Peer Navigator’s skills and abilities and provide Peer Navigation services in a trauma-informed manner that is consistent across the entire cohort.

  • Basic computer skills necessary, and familiarity with Microsoft Office programs is a plus.

  • Willingness and ability to work with people from a variety of racial, cultural, social, ethnic and economic backgrounds with various lifestyles, sexual orientations, and of all ages and genders.

  • Relevant experience in substance use disorder prevention, intervention and treatment and/or with behavioral health (mental health and/or substance use) challenges, with demonstrated stability of at least two years or more.

  • Experience working with vulnerable youth and young adults.
