The University of Miami is currently seeking a Patient Navigator to work at the UM Hospital & Clinics (UMHC) in Miami. The incumbent facilitates patients’ ability to obtain services by identifying barriers to care, coordinating the development of treatment plans, and monitoring the implementation and follow-up of services ordered by caregivers.

Primary Duties and Responsibilities (For Non-exempt Employees Include Percent of Effort):

  • Telephone responsibilities include, but are not limited to speaking to the patient, and contacting a variety of health care facilities to obtain patient medical record information.

  • Explains consent for obtaining outside records, and HIPAA (if applicable) to patient; obtains signatures (may be done via email)

  • Submits signed consent to health care centers as appropriate to initiate retrieval of medical records, pathology slides, x-rays and any other pertinent report

  • Tracks pending records to ensure delivery prior to clinic visit such as pathology reports etc.

  • Communicates to the appropriate SDG Nurse Coordinator, if difficult or unable to obtain a pertinent record

  • Travels within the facility and campus to retrieve records/films/CD/s as needed

  • Prepares pathology slides and submits for consultation

  • Returns pathology slide to appropriate institution or patient

  • Communicates with physician as needed

  • Assembles, completes, and processes patients’ charts as indicated

  • Maintains confidentiality of patient information.

  • Maintains an accurate clinical record, recording information per hospital policy.

  • Performs patient call back to complete Patient Satisfaction Survey.

  • Utilizes UChart and RIS/PACS/Cerner

  • Reports unusual incidents in a timely manner to appropriate supervisory personnel.

  • Assist nursing staff in collaboration of multidisciplinary meetings

  • Assists with other clerical duties, not limited to faxing, scanning, copying

  • Performs other duties as assigned.

    Knowledge, Skills, and Abilities:

  • Excellent customer service skills are required.

  • Strong written and oral communication skills.

  • Knowledge of medical terminology is preferred.

  • Able to handle multiple tasks simultaneously.

  • Ability to establish and maintain effective working relationships with patients and staff

  • Computer skills Microsoft Office

    Education Requirements (Essential Requirements):

  • High School diploma or equivalent
