Atlantic Health System Patient Navigator in Morristown, New Jersey

Overview

Atlantic Health System is seeking a full time Patient Naviagor. Patient Navigator is a committed, engaged professional who primary function is to assist patients in accessing HIV care and services for the first time and or remain engaged in their own treatment.

Responsibilities

Essential Functions:

  • The navigator utilizes best practices to assess the needs of new and out of care patients and connect them with internal services and or community resources.

  • The successful candidate will have the ability to work collaboratively with the Morristown Family Health Center’s Ryan White Program (hereafter referred to as “FHC”) infectious disease providers, medical case managers, nurses, and staff as well as with community agencies to assist clients with the challenges of remaining in care and on treatment.

Qualifications

Education:

  • Bachelor degree in public health, nursing or social work, or related field.

Qualifications/Requirements:

  • Experience (>1 year) in HIV field with patient contact including patient education, and counseling.

  • Experience working with electronic medical records (EPIC experience preferred)
