ICONMA, LLC Case Manager in United States

  • Telephonically (remotely) assessing, planning, implementing and coordinating all case management activities with patients to evaluate the medical needs of the member to facilitate the member’s overall wellness. Develops a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a member’s overall wellness through integration.

  • Utilize assessment skills and discretionary judgment to develop plan of care based upon assessment with specific objectives, goals and interventions designed to meet member's needs and promote desired outcomes

  • Coordinate services between Primary Care Physician (PCP), specialists, medical providers, and non-medical staff as necessary to meet the complete medical needs.

  • Provide patient and provider education.

    Requirements:

  • Holds a Compact Registered Nurse License and should be residing within Florida, Indiana, Missouri, or Tennessee.

  • 3-5 years of hospital Medical/surgical experience.

  • Associate Degree or nursing diploma required.

  • Requested criteria above average computer skills (Microsoft Office suite).

    Preferred:

  • Triage, case management and/or rehabilitation experience preferred.
